All About Me form
If you have been asked to go to a Children’s Hearing, you will be sent a package of information from the Children’s Reporter. This will be posted to the place where you are staying a few days before your Hearing.
In the pack, you will receive a paper copy of a form called All About Me. You can use this form to say how you are feeling, if you are happy with school and where you are staying or if you have any problems.
The pack will tell you when and where your Hearing is, and you can take your All About Me form along to your Hearing, so the Panel Members know how you are.
If you prefer, you can fill this form in online. All you need to do is click on the form below which is best for you (if you are 11 years and under or 12 years and over).

Type in your details carefully. Please make sure you include your name, your date of birth and the date and the place of your Hearing, for example, Glasgow/Edinburgh/Aberdeen.
If you want to keep a copy of the form, please just click print. Once you have completed the form, simply click submit. You will receive an email telling you that we have received your form. We will be able to email you back Monday to Friday 9am to 5pm only.
If you have any problems filling in the form, please contact your Reporter direct (their name and contact details will be on the letter telling you where and when your Hearing is).
